Outline for Filling in the Use of Exchange Room Application

Name of Organization and Name of Applicant
|
|
In the event that the exchange room is being used for an organization or group function, then please write both the organization’s name and the name of the applicant representing the group. In case of individuals wishing to use the room, please write your full name as the applicant together with your Kokusai Hiroba member number.

|
Please include your computer or cell phone e-mail addresses if available so that we may contact you as soon as possible if the need arises.
|
|
|
Desired Period of Use
|
|
1: Use of the room is during hours when Kokusai Hiroba is open and there are no previous activities planned for the room.
2: There is a limit for the use of the room of two times per month.
3: There are three time slots available for use of the room, which includes time for preparations beforehand and cleanup afterward.
(1) 10:00 – 1:00
(2) 1:00 – 4:00
(3) 4:00 – 7:00
※Please consult with us if the time that you wish to use the room is not listed above.

Contents of Meeting
|
|
Please tell us as specifically as possible what the meeting entails. If you can provide an attached proposal, pamphlets, or other information, then there is no need to fill out this portion of the application.
|

Drinks and Snacks
|
|
Bringing drinks and snacks into the Exchange Room is normally restricted, but we do permit them on a case-by-case basis, such as when they are directly involved with the purpose of the meeting (such as during tea ceremony workshops). If you wish to use them, then please be sure to tell us the reason(s) why on the application.

Additional Information

|
Requests to use the Exchange Room are accepted as early as one month prior to the day desired for use. (In the event that this day falls on a day in which Kokusai Hiroba is closed, then the request will be accepted on the first following day we are open.)
